Aetna contracts with a large network of health care providers so that you can find in-network doctors and hospitals in your area. Start by visiting Costcobenefits.com and clicking “Find a Doctor” under Health & Wellness. You can search by name, provider type, condition or procedure. Look for the “Quality & Effective Care” label at the top of a doctor’s profile. It means the provider has met the high standards set by Aetna Smart Compare®.
Aetna Dental offers nationwide access to dental care. Your dental plan covers general dental care, specialist care and orthodontic care from in-network dental providers. To get the most from your plan, make sure to schedule your free routine dental cleanings and exams. And remember, with the Aetna Network Dental plan, you must use in-network dental providers, except for orthodontists and certain emergency care situations. Find in-network dental providers by contacting the Aetna Dental team or downloading the Aetna Health app. As long as you use an in-network dental provider, your dental plan covers these preventive services at 100% with no deductible:

EyeMed administers your vision and hearing aid benefits. The plan will pay up to $80 for your annual refractive eye exam and $175 toward the purchase of prescribed eyewear every year. You also get $1,750 toward the purchase of prescribed hearing aids every 4 years.
Costco Health Solutions (CHS) provides the prescription benefits for your Costco medical plan. You can fill your prescriptions at any Costco Pharmacy or through Costco Home Delivery Pharmacy. When there’s no Costco Pharmacy available, you can go to any pharmacy that accepts your CHS card. For emergencies, you’ll be able to fill your prescription anywhere; however, you may have to pay the full cost and submit a request for reimbursement.

Elect a Health Care Reimbursement Account (HCRA) and/or Dependent Care Assistance Plan (DCAP) during Annual Enrollment. You can also enroll in the Commuter Benefits Plan (CBP) for commuting expenses, such as vanpools and parking. Download the Inspira Mobile app to manage your account on the go. UBS administers the Costco Employee Stock Purchase Plan (ESPP). You can purchase the company’s common stock through regular payroll deductions.
Unum administers Costco’s voluntary short-term disability insurance plan (not available in all states), Salary Continuation, Costco Personal Medical Leave, the Family Medical Leave Act (FMLA) and Bonding Leave, which can be used while you bond with your child following birth or adoption. These programs help replace lost income if you become disabled due to an illness, non-work related injury or other medical condition such as pregnancy. Enrollment in a Costco benefits plan is not required.
Please note: If you live in a state with a paid family and medical leave program, your primary disability coverage is provided under your state’s program. You are also covered by voluntary short-term disability at a lower payroll deduction. Unum will reduce your STD benefit by any amount you are eligible to receive under the state benefit, even if you choose not to file a claim with your state.
Unum administers Costco’s long-term disability insurance plan, providing help with income replacement after you’ve been totally disabled due to a non-work-related injury or medical condition for 26 weeks. LTD insurance is provided automatically at no cost to employees enrolled in a Costco medical plan. As long as you don’t decline all Costco benefits, you can elect LTD insurance and pay a portion of the cost for coverage.
