Taking a leave of absence can be confusing. Costco’s disability insurance plans, administered by Unum, help replace lost income if you become disabled due to an illness, injury or other medical condition such as pregnancy.

Types of leave benefits

Costco partners with Unum’s LeaveLogic to help you understand, plan and manage your leave benefits so you can focus on what matters most. You have several different types of leave available to you depending on your personal situation. LeaveLogic isn’t available yet in Puerto Rico.

Here are some of the main types of leave benefits:

Voluntary short-term disability (VSTD)

VSTD is available in all states except CA, HI, NJ and NY. It’s automatic for eligible hourly employees unless you decline coverage. The cost is a small percentage of your pay. If you live in a state with both a state-mandated program and VSTD, like WA or OR, you’ll pay a lower rate for VSTD.

State-mandated

Some states have their own family and medical leave programs — including HI, CA, NJ, NY, WA and OR. You’ll need to file a claim with your state if you take a leave, even if you have other leave benefits.

Family & Medical Leave Act (FMLA)

FMLA is a federal law offering 12 weeks of protected unpaid leave to care for a seriously ill spouse, child or parent.

Bonding leave

If you’re having a baby, you also have access to paid bonding leave. A new baby means time away from work for the birth parent to recover, plus additional time to bond with your new child.

Costco offers paid bonding leave to give employees (moms and dads) dedicated time off to be with your new child — whether you gave birth, adopted or became a foster parent. Bonding leave takes place immediately after the recovery period for the birth parent, or within 1 week of your child’s arrival.

LeaveLogic can help you plan your leave

You may be eligible for several types of leave, and they may run at the same time, or overlap. Your VSTD coverage will be reduced by what you’d receive from a state-mandated program, even if you don’t file a claim with your state. To receive all the benefits available to you, you’ll need to file a claim with your state.

Take the guided tour.
Review your timeline, to-do list, benefits and profile.

When you’re ready

File your leave with Unum and notify your manager. You can file with Unum through LeaveLogic. Unum handles administration of your leave of absence, so you’ll manage your leave using the Unum app, at Unum.com or by calling 877-403-9348.
